In February of 2011 CarePartners launched an exciting new program with the South West CCAC that combines technology with compassionate, high quality home care to create greater support for clients who wish to spend their final days at home.
Six personal support workers have joined CarePartners (a first for our traditionally 'all nursing' agency) and have participated in education that enhanced their ability to support clients who require end-of-life care. As a result CarePartners has awarded them the designation of e-PSWs.
The e-PSW provides palliative care overnight in the home of the client and is supported by a CarePartners delegating nurse who is at a remote site. The e-PSW uses an I-Phone and specialized software to communicate with the nurse; the nurse uses a computer to monitor the client's status and direct the e-PSW. To date, clients and families report a high level of satisfaction with this program.
The program has started in Owen Sound but CarePartners is already working with the SW CCAC to expand the program outside of the town's borders as demand for the service grows.
