    CarePartners is a leading provider of home nursing services throughout Ontario and is pleased to announce the recent acquisition of Dynamic Therapy Solutions – an excellent therapy provider serving the Huron, Perth and Waterloo Wellington regions.

    Dynamic Therapy Solutions began in 1998 in the Guelph area with 4 Physiotherapists servicing clients in the community. Since then the company has grown to 44 health care professionals comprised of  Physiotherapists, Occupational Therapists, Speech Language Pathologists and Dieticians, providing services for the Waterloo Wellington CCAC and SouthWest CCAC. Dynamic Therapy Solutions is dedicated to client centred care in the provision of high quality therapy services based on best practice, innovation and excellence. Service environments include homes, schools, workplaces and long term care facilities.

    CarePartners began in 1984 in the Wingham area near Lake Huron in Southwestern Ontario. The company started with 3 nurses servicing a very rural region providing services 24 hours per day and seven days per week.   • CarePartners focuses on Patient Safety

    CarePartners has always been committed to the well-being of its clients but June represents a new step forward for CarePartners in terms of patient safety. As part of our work to meet the standards of Accreditation Canada, CarePartners is launching a package of initiatives including:

        •a Medication Reconciliation and Management Program 
        • a Falls Prevention Strategy 
        • a Home Safety Risk assessment Program

    Research shows that reviewing a client's medication and making sure that all the medications are being taken correctly increases the likelihood that client's will not miss medications, will not duplicate medications and will take the right dose - all important pieces to ensuring that medications help rather than harm clients. Research also shows that home care nurses can play a valuable role in increasing the safety of clients by identifying clients at risk for injuries related to home hazards and falls and by providing education to the client who is at risk for falling and his family as well as referral to other health professionals.

    The toll of medication errors and injuries in the home on the health care system is significant and CarePartners nurses will be ensuring that they are providing their clients with the best possible information and support to keep them safe.

  • CarePartners BPG Journey Enters Year 3

    CarePartners is entering its third and final year as an RNAO Best Practice Spotlight candidate and there is much to celebrate! Four RNAO Best Practice Guidelines have been implemented at pilot sites - these guidelines focus on the prevention of pressure ulcers, the management of ostomies, the management of pain and the care of clients with diabetic foot ulcers. Nurses are using their new knowledge of best practices to achieve better outcomes for their clients and to better manage our limited health care resourcses.

    In addition to improvements in clinical practice, nurses have participated in RNAO champions workshops, taken part in Advanced Clinical Practice Fellowships and presented at conferences.

    CarePartners participation in the RNAO Best Practice Spotlight organization candidacy has resulted in tremendous organizational learning and has significantly developed CarePartners capacity to deliver the high quality nursing care that defines us as an agency.

  • CarePartners launches E-Shift

    In February of 2011 CarePartners launched an exciting new program with the South West CCAC that combines technology with compassionate, high quality home care to create greater support for clients who wish to spend their final days at home.

    Six personal support workers have joined CarePartners (a first for our traditionally 'all nursing' agency) and have participated in education that enhanced their ability to support clients who require end-of-life care. As a result CarePartners has awarded them the designation of e-PSWs.

    The e-PSW provides palliative care overnight in the home of the client and is supported by a CarePartners delegating nurse who is at a remote site. The e-PSW uses an I-Phone and specialized software to communicate with the nurse; the nurse uses a computer to monitor the client's status and direct the e-PSW.  To date, clients and families report a high level of satisfaction with this program.

    The program has started in Owen Sound but CarePartners is already working with the SW CCAC to expand the program outside of the town's borders as demand for the service grows.

    CarePartners is thrilled to be part of this innovative venture that allows us to expand our broad range of home care programs and services.

  • CarePartners welcomes Rainbow

    In the summer of 2010 CarePartners began delivering nursing services in Parry Sound.  This was CarePartners first venture into Northern Ontario. 

    With the excitement of delivering nursing services in Parry Sound, CarePartners began to search for ways of expanding our services to the North East CCAC.  This search led CarePartners to an excellent nursing provider, Rainbow Providers Inc. On December 1, 2010 CarePartners purchase of Rainbow was finalized.

    Led by Mary and Jack Birtch, Rainbow Providers has been delivering high quality nursing services in the South Temiskaming District for 13 years. CarePartners welcomes Rainbow and its staff!
